Storing Your Health Care Proxy for Fast Access

A copy of an executed health care proxy can someday be needed in a hurry. You should make sure that whoever you have appointed as your "Agent" has at least one copy and keeps it in a readily available place, such as the glove compartment of a car. (The form allows for the use of photocopies with the force of the original.)

To make sure that your health care Agent under your health care proxy can be contacted in case you are ever rushed to a hospital, we suggest you consider placing a copy on file with the U.S. Living Will Registry. This organization will send you stickers to place on your drivers license and health insurance card to alert the hospital that you have a health care proxy and the address and phone number of your Agent or Agents.

Should you be taken to a hospital in an emergency, the sticker will serve as notice to the hospital that your documents are on file with the U.S. Living Will Registry. Upon the hospital's request, the organization will fax your document to the hospital, wherever you are in the U.S.
